Memory Book

 

We are prepared to put together a Memory Book for the reunion, but we need someone to volunteer to put it together once the questionnaires have been returned.

 

 

 

Everyone who returns their questionnaire will be sent a copy of the finished book.  Help make this one the best one yet!

Please complete your Memory Book Questionnaire and return it by [date].

3 Ways to do your questionnaire:

  1. Online - Fill out questionnaire and submit it immediately
  2. MS Word document - Download, then
    • print it, write answers, and mail; or
    • type in answers, email it
  3. Download as PDF file; print; write answers; mail.

 

Note: This is different from the info on your Alumni Page.  The Memory Book will be printed, and mailed to participants.  None of the Memory Book info will be posted on the website.
